  • Zoe Snape

“After qualifying as a Registered General Nurse, I worked in critical care areas at Hope and Wythenshawe Hospital. I first worked in Primary Care in 2006 as an Active Case Manager and latterly I was a Community Matron. My role involved working with patients to manage their complex, long term conditions, with the aim of reducing hospitals admissions.

After a brief role as a District Nurse I was offered my current role as a Practice Nurse, here at Beech House in June 2011.I feel this was a natural progression for me, as I thoroughly enjoy Primary Care, disease management and prevention.

I am a qualified Independent Nurse Prescriber who can prescribe a variety of medicines and I am starting to learn the wide variety of Practice Nurse roles, such as immunisations, vaccinations, Smears, Chronic Disease management etc. I will also be commencing an Advanced Nurse Practitioner Masters course in September 2011.

I am married, with two sons, two dogs and three cats. I have always (well as long as I can remember) wanted to be a nurse and I feel privileged to be able form strong relationships with my patients and be part of a team that can help improve their health and well being”
