Registering

Please note all new baby registrations are automatically completed at the surgery from the maternity discharge summary sent to us. You should recieve a phonecall from us a few days after discharge from hospital to confirm your baby’s details. If you don’t hear from us after one week of discharge please ring us. Your baby can still be seen at the surgery before we ring you.

We welcome new patients to register at our practice.

If you wish to register at our practice you need to complete a registration form and new patient questionnaire, these are available at reception.  For your convience you can also register online by clicking here.

The form will ask you personal details such as your name, address and place of birth please complete all relevant details on the form.  If you have your NHS number, please include this on the registration form, the information is needed to trace your medical records and send them to your new doctor.

Once you have completed and signed the form you will be automatically placed on the nominal list of the doctor currently assigned patients, which changes each month. This ensures continuity of care and a named doctor responsible for your care.

Once your forms have been processed and you are registered at Beech House Surgery you will need to book a new patient screening appointment with our practice nurse/HCA.  This enables us to take relevant details of any past /current medical conditions, medications and provide a simple health screening.  You will be required to provide a urine sample for this appointment.

This check is not required for new patients under16 years of age.

The Primary Care Trust will send you a new medical card with your new doctor’s details on for you to keep.

On the back of your registration form you are given an option to go on a central register as an organ donor if you wish to do so.
