How do i Ask the Practice to Complete a Form?

If you require a doctor to complete a form please leave it with the receptionist at the front desk or by posting it to the surgery. There is often a charge for completing forms which can sometimes be claimed back through insurance companies and employers.

Please allow a week before telephoning the surgery to see if the form has been completed. Forms are completed in the doctor’s own time and are completed as soon as they can.
